Uninsure Services
Uninsured services and all forms requested by third parties are not included under the Provincial Health Insurance Plan and will incur charges to the patient. The rates for these services are established by the Ontario Medical Association and are applied accordingly.
We provide a “block fee” payment option that encompasses or discounts uninsured services on an annual basis. This “block fee” serves as a substitute for individual payments for uninsured services. For your convenience, we accept cash, debit, cheque, VISA, and MASTERCARD
A fee of $25.00 will be charged for all NSF cheques.
If you have not received a block fee letter, please contact the clinic.
Renewals of prescriptions via telephone require a period of three business days for completion. Kindly verify that you possess an ample amount of medication or secure your prescription during your visit to the office
